Practise an adaptable sequence
First practise each named action manually, then link actions while watching the target, your resources and the group. Stop the damage sequence when movement or recovery is needed. Keeping those decisions under your control makes it easier to understand a failed encounter.
Change the plan for PvP
Against a player, watch whether a debuff has been removed and whether a control has already been used. Avoid spending every response at once. Keep room to move and decide which ally or enemy actually needs your attention; an automatic sequence cannot make that choice for you.
